6. The significant of the study
This research proposal will test the inter- and intra-rater reliability and content validity of the Thai translated version of the Palliative Performance Scale (PPS). PPS is a functional status tool used in palliative care for communication, survival prediction and workload. After initial translation into Thai, PPS has been anecdotally used with success in Maharaj Nakorn Chiang Mai hospital under supervision of Dr. Laddawan and Ladarat over the past 12 months. It may provide significant value for palliative patients in assessing performance status and prognosis. PPS is very useful in communicating about patients and for the prediction of survival of palliative care patients. PPSv2 is a new tool in medical schools and nursing in Thailand. Maharaj Nakorn Chiang Mai hospital is the first hospital to use PPSv2 in Thailand for assessing performance status of the palliative care patients. The palliative care patients at Maharaj Nakorn Chiang Mai hospitals has 3 groups : the incurable and serious illness patients who tend to be fatal, the cancer patients in every stages, the end of life patients.
Mahraj Nakorn Chiang Mai hospital has established the core palliative care model based on PPS level. Moreover, Maharaj Nakorn Chiang Mai hospital has a palliative care Collaborative Care Plan (CCP) modified from the palliative care CCP of Queen's university. This palliative care CCP has given care for the palliative care patients 3 categories as PPS level : PPS level 70% to 100 %, PPS level 40% to 60% and PPS level 0% to 30% . We determine palliative care CCP as PPS 3 categories for the quality of care. PPSv2 is very useful in palliative care system of Maharaj Nakorn Chiang Mai hospital. PPSv2 is the origin of Palliative care model of Maharaj Nakorn Chiang Mai hospital by the supporting of Michael G.Downing .
In conclusion, PPSv2 is very useful in palliative care system of Maharaj Nakorn Chiang Mai hospital. PPS is the origin of Palliative care model of Maharaj Nakorn Chiang Mai hospital.
Although PPSv2 demonstrated as a reliable and valid tool in abroad, the translation of PPSv2 to the Thai language needs to be confirmed as reliable and its value tested as a clinical tool for palliative care patients.
Therefore it is necessary to test the reliability and validity of PPS in Thai version which is called PPS Adult Suandok.
The study will take approximately 12 months to complete and prepare for publishing. Funds have been approved by the Victoria Palliative Research Network.
7. The objectives of the study
7.1 To examine the inter- and intra-rater reliability of Thai PPS tool by clinicians, with a set of simulated palliative care patient case scenarios, in assessing their performance status.
7.2 To examine the validity of the Thai PPS by content validation and surveying clinicians on its purpose and usefulness.
